Basic Forms
4-H'ers must complete a new Enrollment Form and Code of Conduct each year. This one copy will cover all events (virtual or in-person) that occur between August 1, 2024 - July 31, 2025. A new Medical Release Form and Medicine Form (if needed) are required for each event.

Virtual Program Forms
4-H'ers who participate in online or virtual programs must complete both the Electronic Consent and Participation Agreement/Waiver form. These forms will cover all online events that occur between August 1, 2024 - July 31, 2025.
