These forms must be completed when joining 4-H.
Forms are also availabe at the 4-H Office.
Important Information:
- Breakfast will not be provided for any summer activity. Make sure your child eats before drop-off.
- Lunch is provided.
- Children may be dropped off at the 4-H Office up to 30 minutes before start time.
- Late pick-ups will result in additional fees: $5 for every 5 minutes past pick-up time.
- Time adjustments will be made as needed and will be communicated to parents.
- You must fill out a Code of Conduct & Medical Release before your child can participate in 4-H programs.
- 4-H is not responsible for lost or stolen items. Leave anything valuable or irreplaceable at home.
- Label all personal items with child’s name. Unclaimed belongings will be donated or disposed of August 1st.
- Space is limited. Register NOW!
- Registration and payment due no later than 7 days before each activity.
- Drop off & pick up will be at the 4-H Office, unless stated* otherwise.
