Enrollment and Forms

We are excited about the programs and activities that we offer to our youth and welcome new members at any time during the year. So, don't be shy, come and join us to create some of the best memories.  Once a student is enrolled they may register for events, clubs, and programs.

4-H program enrollment, activities and trips require the following forms to be filled out and on record. 4-H'ers must complete a new Enrollment Form and Code of Conduct each year.  This one copy will cover all events and activities (virtual or in-person) that occur between August 1, 2026 - July 31, 2027.

 

A Medical Release Form and Medicine Form (if needed) are required yearly for each club.  A new Medical Release Form and Medicine Form (if needed) are required for each event (Project Achievement, Junior Conference, Fall Forum, etc).

 

Alternative travel to or from a 4-H event and not with the county group.
