Forms for Events
The following forms are needed on file for 4-H'ers attending out-of-school events.
- 4-H Enrollment (New copy needed each year)
- Medical Release (New copy needed each year)
- Code of Conduct (New copy needed each year)
- Virtual 4-H Program Agreement (New copy needed each year)
- Virtual 4-H Program Consent (New copy needed each year)
- Medicine Form (Only if medication must be administered)

Who Is Eligible?
Any youth ages 9-19
Cloverleafs: 5th-6th grade
Juniors: 7th-8th grade
Seniors: 9th-12th grade
Does It Cost Anything To Join?
4-H membership is free; however, some optional activities may charge a fee.
