Georgia Pesticide Licensing
Visit the Georgia Pesticide Licensing website to get directions on enrolling into the testing site and access forms such as private pesticide applicator’s licensing registration form, and a verification of lawful citizenship affidavit (which has to be filled out by the participant, signed, and notarized). Participants will need to provide a photocopy of identification such as a driver’s license or passport.
Once the participant has finished the testing, he/she will need to print out a completion form. This form, along with the application verification of lawful citizenship affidavit and copy of his identification, will need to be sent to the Georgia Department of Agriculture.
