Forms & Downloads

Basic Forms 

4-H'ers must complete a new Enrollment Form and Code of Conduct each year.  This one copy will cover all events (virtual or in-person) that occur between August and July. A new Medical Release Form and Medicine Form (if needed) are required for each event.
