Georgia Pesticide Licensing

Visit the Georgia Department of AG Pesticide Licensing website to get directions on enrolling into the testing site and access forms such as private pesticide applicator’s licensing registration form, and a verification of lawful citizenship affidavit (which will has to be filled out by the participant, signed, and notarized). Participants will need to provide a photocopy of identification such as a driver’s license or passport.

Once the participant has finished the testing, he/she will need to print out a completion form. This form, along with the application verification of lawful citizenship affidavit and copy of his identification, will need to be sent to the Georgia Department of Agriculture.
