4-H Required Forms

Lowndes 4-H requires certain forms to be kept on file for each child who wishes to participate. These must be renewed every school year before a child will be able to participate in any 4-H activities, virtual or otherwise.  

You will need to complete the online enrollment form and Follow Lowndes County 4-H on Facebook. When your child participates in an activity, you will  complete the Medical Information Release Form and email the form to our office at grace.chauncey@uga.edu or print and drop off at 2102 E  Hill Ave, Valdosta GA 31601.

Required Youth Participant Forms:

• Enrollment Form (For grades 4th-12th to sign up for 4-H)
  
• Medical Information Release Form (Should be completed 1-3 weeks prior to event or activity)
• Code of Conduct (Emailed to you when your child signs up for an activity or event)
