4H required forms

Brooks 4-H requires certain forms to be kept on file for each child who wishes to participate. These must be renewed every year and be filled out before a child will be able to participate in any 4-H activities, virtual or otherwise. Please be sure to fill them all out and email them to us at uge4027@uga.edu or print them out and drop them off or mail them to 400 East Courtland Ave Quitman, Ga 31643.
