4H required forms

Brooks County  4-H requires certain forms to be kept on file for each child who wishes to participate. 

These must be renewed every year and completed before a child can participate in any 4-H activities.

 Please be sure to fill out these forms; you can 

  • Email them to us at uge4027@uga.edu
  • Print them out and drop them off at the UGA Brooks County Extension Office 
  • Mail them to 400 E Courtland Ave. Quitman, Ga 31643
