4-H Forms

A completed 4-H Enrollment Form  is required for all students new to Columbus-Muscogee County 4-H.

A  4-H Medical Form must be completed before each event a student attends.

A  4-H Code of Conduct Form must be completed every 4-H year (August 1-July 31).

 

If your student will need to take any medication during an event (prescription or over-the-counter), a 4-H Medicine Form must be completed before the event.
