4-H enrollment is open year round. We welcome new members to join us. Enrollment forms can be completed here and emailed back to us at uge3283@uga.edu.

4-H activities and trips require the following 3 forms to be filled out and on record. Check with your 4-H Leader to make sure you have the proper paperwork before leaving on your activity. For questions please call 912-529-3766.

Consent Form

Georgia 4-H county, district, and state 4-H offices are providing various 4-H programs electronically (e.g., through phone or designated video platform) throughout the 4-H program year beginning August 1, 20__ to July 31, 20__. Your online information has been collected in order to obtain your consent for your child’s participation in the Program. In the event your consent is not provided within a reasonable time, your online contact information will be deleted. In order for this form to be valid it must be printed, signed and returned to the 4-H/Extension office. For questions please call 912-529-3766.
