Madison County Master Gardener Extension Volunteer Information
Listed below are required forms necessary for Master Gardener Extension Volunteers (MGEV's) serving in Madison County. Included is explanation for each form to help prevent confusion.
MGEV Memorandum of Understanding - This form should be signed and returned to the Extension office each year by returning active MGEV's.
MGEV Agreement Form - This form should be signed and returned to the Extension office each year by returning active MGEV's.
MG Volunteer Activity Report - This form should be completed if a MGEV participates in an educational activity where he/she is directly involved in providing education. This information is used for reporting by the local Extension office and should be turned in to the Extension office at the end of each month an activity takes place.
MGEV Activity Proposal Form - This form should be filled out by any MGEV proposing a new project or activity that is in addition to current MGEV projects. Any new projects must be approved by the Extension office in order for hours to count towards annual reporting.
MGEV Logbook (Includes Annual Summary Form) - The logbook should be filled out by all MGEV's during the course of the year and should be summarized on the annual summary form and returned to the Extension office at the end of each year. Please return logbook and summary forms to the Extension office.
MGEV Consultation Report - This form should be used when any home visits or consultations are made with clients. Whenever possible, prior contact should be made with the Extension office when making a home visit.
